  • Accessibility and validation of survival data in survival curve for meta-analysis

    Hazard ratio (HR) is usually regarded as the effect size in survival studies. Meanwhile, it is supposed to be perfect for pooling results in the meta-analysis of survival data. However, it does not function usually due to absence of original data for pooling HR. As a compromise method, entering data from reading Kaplan-Meier curves and follow-up times into the calculation spreadsheet can also be used to obtain related survival data. But related study on the subject is scarce, and opinions are inconsistent. Accordingly, we conduct this study to further illustrate the procedure in details.

  • Inhibition effect of minocyclinye on pressureinduced apoptosis of retinal neuron cells cultured in vitro

    Objective To observe the effects of minocycline to the viability and apoptosis of ratprime;s retinal neuron cells (RNC) under pressure, and to investigate the neuroprotective mechanisms of minocycline against the RNC damage. Methods Establish a model of ratprime;s RNs under pressure cultured in vitro, the protective effect of minocycline is observed by different methods, including observing the morphology of the cells, evaluating the cellsprime; viability by methyl thiazolyl tetrazolium (MTT) colorimetry assay, and detecting the cellular apoptosis with acridine orange/ethidium bromide (AO/EB) double staining by fluorescence microscopy. Immunocytochemistry was used to detect the expression of iNOS and caspase-3 in the cells. Results Obvious morphology changes of RNC were found in cells under pressure compared with the control; the viability of RNC decreased and cellular apoptosis was found in 53.93% cells. The cellular morphology improved in the cells treated by 20 mu;mol/L minocycline, the cellular viability significantly increased, and the cellular apoptosis was found in 17.29% cells. In addition, the expression of iNOS and caspase3 in the treated cells decreased compared with which in the pressured group. Conclusion Minocycline with a certain concentration can effectively inhibit pressureinduced damage and apoptosis of RNC of rats, and the inhibitory effect on expression of iNOS and capases-3 may be the underlying mechanism.

  • Association of Overweight and Obesity with Type 2 Diabetes Mellitus: A Meta-Analysis of Cohort Studies

    Objective To explore the correlation between overweight, obesity and incidence of type 2 diabetes mellitus (T2DM). Methods Databases including PubMed, EMbase, The Cochrane Library (Issue 3, 2012), CNKI, VIP, CBM and WanFang Data were searched from inception to May, 2012 to collect the cohort studies on the correlation between overweight, obesity and incidence of T2DM. The studies were screened according to the inclusion and exclusion criteria by two researchers independently, the quality was evaluated, the data were extracted, and then meta-analysis was performed using RevMan 5.1 and Stata 11.0 software. Results A total of 8 studies involving 101 864 participants were included. The results of meta-analysis showed that, compared with the normal weight population, the onset risk of T2DM was obviously higher in the overweight (RR=2.59, 95%CI 2.11 to 3.19, Plt;0.000 01), and obese (RR=6.28, 95%CI 4.99 to 7.91, Plt;0.000 01) populations. In the subgroup analysis, the onset risk of T2DM was higher in the western obese population (RR=6.91, 95%CI 5.59 to 8.56) than the eastern obese population (RR=4.19, 95%CI 2.93 to 5.99). Based on the Grading of Recommendations Assessment, Development and Evaluation (GRADE), the quality of the included studies on T2DM developed by overweight and obesity was low and medium respectively. Conclusion Overweight and obesity can increase the onset risk of T2DM which is higher in the female and western obese populations than the male and eastern obese populations respectively.

  • Experiments study on mechanical behavior of porcine lumbar intervertebral disc after nucleotomy under compression

    In order to study the mechanical behavior of degeneration and nucleotomy of lumbar intervertebral disc, compression experiments with porcine lumbar intervertebral discs were carried out. The lumbar intervertebral discs with trypsin-treated and nucleus nucleotomy served as the experimental group and the normal discs as the control group. Considering the effects of load magnitude and loading rate, the relationship between stress and strain, instantaneous elastic modulus and creep property of intervertebral disc were obtained. The creep constitutive model was established. The results show that the strain and creep strain of the experimental group increase significantly with the increase of compression load and loading rate, whereas the instantaneous elastic modulus decreases obviously, compared with the control group. It indicates that the effect of load magnitude and loading rate on load-bearing capacity of intervertebral disc after nucleotomy is larger obviously than that of normal disc. The creep behavior of the experimental group can be still predicted by the Kelvin three-parameter solid model. The results will provide theoretical foundation for clinical treatment and postoperative rehabilitation of intervertebral disc disease.

  • Clinical observation of expanded curettage in treatment of chondroblastoma

    Objective To explore the effectiveness of expanded curettage in the treatment of chondroblastoma. Methods The clinical data of 37 patients with chondroblastoma who were treated with expanded curettage between January 2011 and May 2016 were retrospectively analyzed. There were 24 males and 13 females, with a median age of 17 years (range, 12-30 years). There were 32 primary patients and 5 recurrent patients. Local pain was the first symptom in all patients. The average disease duration was 4.9 months (range, 2-8 months). The lesions were located in the distal femur in 10 cases, the proximal femur in 7 cases, the proximal tibia in 9 cases, the proximal humerus in 5 cases, the patella in 2 cases, the talus in 1 case, the calcaneus in 1 case, and pelvis in 2 cases. According to the Enneking staging of benign bone tumors, all tumors were rated as the 3rd stage. The length of the lesion ranged from 1.2 to 6.9 cm (mean, 3.2 cm). The lesions involved the epiphyseal plate in 19 cases. Results All incisions healed by first intention, and no complications occurred. All patients were followed up 12-76 months, with an average of 40.5 months. At last follow-up, the Musculoskeletal Tumor Society (MSTS) score was 27.5±1.4, and the difference was significant when compared with pre-operative value (18.5±1.9) (t=23.462, P=0.000). The chondroblastoma recurred in 1 case (2.7%) after 5 months. X-ray film showed that bone resorption was found in 6 cases, but there was no obvious collapse in the articular surface of bone graft. The limb shortening deformity occurred in 3 cases who were epiphyseal plate involvement patients and lesions located around the knee joint. But there was no varus deformity, and knee joint activity was not affected. Conclusion Expanded curettage has advantages of low incidence of recurrence and skeletal deformity, good limb function, and it is one of the ideal options for chondroblastoma.

  • Incidence of suicidal ideation of medical students in mainland China over the past ten years: a meta-analysis

    Objective To estimate the incidence of suicidal ideation of Chinese medical students in mainland China over the past ten years by meta-analysis, so as to provide references for suicide prevention and mental health education in Chinese medical students. Methods PubMed, CNKI, WanFang Data and CBM databases were searched for studies investigating the prevalence of suicidal ideation among Chinese medical students in mainland China from January 1st, 2007 to 31st May, 2017. After study selection according to inclusion and exclusion criterion, data extraction and quality assessment, meta-analysis was conducted using Stata 12.0 software. Heterogeneity was evaluated by performing subgroup analyses and meta-regression. We also used sensitivity analysis to determine the influence of individual studies on the overall prevalence estimates. Results A total of 30 studies with 58 757 participants were included, the individual-study incidence of suicidal ideation ranged from 0.84% to 41.35%, with significant between-study heterogeneity detected (I2=99.1%, P<0.001), and the pooled incidence by random-effects model was 12.51% with 95%CI 9.72% to 15.60%. Univariate and multivariate meta-regression revealed that instruments were related to heterogeneity (P<0.01), but other study-level covariates did not contribute to heterogeneity (P>0.1). Conclusion The incidence of suicidal ideation among medical students in mainland China is relatively high, calling more attention to this concern and actions in response for suicide prevention and mental health education in Chinese medical students.

  • Effectiveness and safety of anti PD-1/PD-L1 combined with chemotherapy in the neoadjuvant therapy of resectable non-small-cell lung cancer: A single arm systematic review

    Objective To systematically evaluate the efficacy and safety of neoadjuvant anti PD-1/PD-L1 combined with chemotherapy of resectable non-small cell lung cancer (NSCLC). Methods We conducted a deep search in different databases such as PubMed, EMbase, The Cochrane Library, CNKI , and Wanfang data to identify relevant studies on anti-PD-1 /PD-L1 combined with chemotherapy for resectable NSCLC from inception to March 2023. Two reviewers independently screened the literature and extracted the data to evaluate the risk of bias in the included studies. Then, a meta-analysis was conducted by RevMan 5.4 software. Results Twenty-six studies with 965 patients were included in this meta-analysis. The most relevant results of the meta-analysis showed that the pooled pathologic complete response, major pathologic response, and objective response rates as well as, partial response, operability and R0 surgical resection rate of neoadjuvant anti-PD-1/PD-L1 combined with chemotherapy were 39% [RD=0.39, 95%CI (0.31,0.47)], 59% [RD=0.59, 95%CI (0.53,0.65)], 72% [RD=0.72, 95%CI (0.65,0.80)], 62% [RD=0.62, 95%CI (0.56,0.69)], 86% [RD=0.86, 95%CI (0.81,0.92)], and 94% [RD=0.94, 95%CI (0.92,0.97)], respectively. In terms of safety, the rate of adverse effects (AEs) was 65% [RD=0.65, 95%CI (0.52,0.78)], and the rate of grade 3 to 5 AEs was [RD=0.16, 95%CI (0.10,0.23)]. Conclusion The combination of neoadjuvant anti-PD-1/PD-L1 with chemotherapy has good levels of efficacy and safety in the treatment of patients with resectable NSCLC.

  • Construction and application of integrated management for patients with pulmonary nodules/lung cancer based on “internet plus” and health service team of treatment, nursing, and care

    Lung cancer is the malignant tumor with the highest incidence and mortality rate in China. Early diagnosis and treatment are key to improving the survival rate and reducing the mortality rate for lung cancer patients. This article introduces the integrated management model for patients with pulmonary nodules/lung cancer developed by West China Hospital of Sichuan University based on “internet plus” and health service team of treatment, nursing, and care. The Integrated Care Management Center has established a multidisciplinary team, using internet platforms and artificial intelligence tools to develop a whole life cycle health service system for patients with pulmonary nodules/lung cancer, which is from the screening of high-risk population for lung cancer, the intelligent risk stratification and follow-up management of pulmonary nodules, the subsequent standardized diagnosis and treatment of lung cancer and comorbidity management, until the patient’s demise. After the implementation of this model, the malignancy rate in surgically treated patients with pulmonary nodules reached 85.08%, and the patient satisfaction score was 95.76. This model provides a new idea and reference for the innovation of chronic disease service model and the management of pulmonary nodules and lung cancer.

  • Development and implementation of genetic re-education project for neurologists

    Objective To explore and implement a systematic, case guided online interactive training course for neurologists to improve their diagnosis and treatment of rare genetic diseases. Methods Doctors who participated in the course investigation of the neurogenetic project of the Department of Neurology of Peking Union Medical College Hospital between January and September 2021 were selected. Based on andragogy theory, a genetics training course for neurologists was developed by applying Kern’s six steps of curriculum development. According to the time of participating in the doctor’s courses, they were divided into three groups: completed all courses (10.7 h group), completed more than 1/2 courses (5.3~10.7 h group) and completed less than 1/2 courses (<5.3 h). According to the length of service, they were divided into groups of less than 10 years, 10-20 years and more than 20 years. Analyze the benefit difference of different doctors’ training time, and collect their feedback scales on the curriculum for the improvement of follow-up courses. Results A total of 54 doctors were included. Among them, 17 (31.5%) completed all courses, 29 (53.7%) completed more than 1/2 courses, and 8 (14.8%) completed less than 1/2 courses. There was a statistically significant difference among the three groups in the self-assessment improvement score (H=12.341, P=0.002). The results of pairwise comparison between groups of self-assessment improvement score showed that the <5.3 h group was lower than that of the 10.7 h group (P=0.007), and the the <5.3 h group was also lower than that of the 5.3~10.7 h group (P=0.002). 33 (61.1%) in the less than 10 years group, 16 (29.6%) in the 10-20 years group, and 5 (9.3%) in the more than 20 years group. There was no correlation between participating in work and course time (rs=0.113, P=0.418). 54 (100.0%) believed that they had more than moderate help (≥3 points). Most doctors (>90%) had a good evaluation of the curriculum. Conclusion The periodic neurogenetic re-education project is helpful for clinical diagnosis and treatment of rare neurogenetic diseases.
